Medicare Supplement (Medigap) Plans by State

Medigap benefits are the same for every insurer in a given plan letter, but the rules around them are not. Each state sets its own rating method, its own guaranteed-issue rights beyond the federal minimum, and its own consumer protections. That is why the same Plan G can cost very different amounts depending on where you live. Use the table below to find your state and jump to the detailed guide.

Quick Facts Across All States

  • 11 jurisdictions use community rating (same premium regardless of age).
  • 1 use issue-age rating (premium tied to the age you enroll).
  • 39 use attained-age or mixed rating (premium rises as you age).
  • Birthday Rule states in our data: California, Oregon. These let existing policyholders switch plans each year without health questions, within a set window after their birthday.
  • Estimated Plan G premiums at age 65 range from about $110/month in Arkansas to about $260/month in New York.

How to Use This Guide

  1. Start with your state's rating method. It determines whether your premium will climb with age.
  2. Check for Birthday or Anniversary Rules. These are valuable if you ever want to switch to a cheaper insurer without medical underwriting.
  3. Choose a plan letter first, then compare insurers. Because benefits are identical, price and rate-increase history are what differ.
  4. Confirm with a neutral source. Your state's SHIP offers free counseling at shiphelp.org.

Frequently Asked Questions

Why do Medicare Supplement premiums differ so much between states?

Benefits for each plan letter are standardized by federal law, but premiums are set by insurers using local healthcare costs, competition, and each state's rating rules. A Plan G that costs roughly $110/month at 65 in Arkansas can cost about $260/month in New York in our estimates, even though the coverage is identical.

Which states use community rating for Medigap?

In our dataset, 11 jurisdictions use community rating, where everyone pays the same premium for the same plan regardless of age. Under attained-age rating (the most common method), premiums rise as you get older, and under issue-age rating your premium is set by the age you were when you enrolled.

Do all states follow the standard Plan A through N lettering?

No. Massachusetts, Minnesota, and Wisconsin use their own standardized Medigap structures instead of the federal lettered plans. If you live in one of these states, contact your state insurance department or SHIP for the plan options available to you.

Can I buy a Medigap plan in a different state from where I live?

Generally you buy a Medigap policy in the state where you live, and the rules of that state apply. If you move, contact your insurer or a SHIP counselor, because your options for keeping or changing a policy depend on state law.
